Filling out the form above is not the complete waiting list application. This form is an initial contact form so we know you are interested. You must download our hard copy application via PDF and submit it to asydor@heritagecare.org once completed.   You must also complete a $70 wait list payment.  This can be completed through the Paypal link below.  Once everything is submitted, you will receive a confirmation that you have joined the waitlist. Please note, the waitlist policy and enrollment procedure is further explained in the "Waitlist Agreement" below.  We expect that all interested families read and understand this agreement before proceeding. HLC is not responsible for lost spots due to a failure to adhere to this agreement.
